In signing our contract you give permission for me to store this information while it is relevant to our work together, to be used in case of an emergency. If you wish to withdraw this permission at any time, please let me know either in person or by email at cathy@cathyrorchardpsychotherapy.com. You have the right to ask to see any information about you that I hold. You also have the right to ask me to correct any information you believe to be wrong. If you have any concerns about how your information is being held, please discuss this with me.
